Enrollment Requirements

All uninsured members of a family, household, business or affinity group (for instance a club or church) must enroll. To exclude a family member from enrollment, please provide proof of that member's insurance.

Fees for Primary Care

$50.00 each, per month, for the first two enrollees, $40.00 each per month, for 3rd and up enrollees, $35.00 each child 0-18 years old (with at least one adult enrolled or $45 without an adult enrolled)

Additional Fee

$20.00 each ($10.00 per child under 18), per month for Extended Laboratory and X-Ray

Automatic Payments

DirectcareMD accepts payments ONLY by a pre-authorized, recurring charge to a single credit or debit card. This fee will be processed after each month the agreement is in force.
