Enrollment Requirements                                                       All uninsured members of  a household must enroll.

Fees for Primary Care                                                                   $45.00 each, per month, for the first two enrollees,                 $40.00 each per month, for 3rd and up enrollees,                   $30.00 each child 0-18 years old (with at least one adult enrolled)

Additional Fee                                                                                  $10.00 each, per month for Extended Laboratory and X-Ray

Automatic Payments                                                                     DirectcareMD accepts payments ONLY by a pre-authorized, recurring charge to a  single credit or debit card. This fee will be processed after  each month the agreement is in force.
